The Handsome Health Health Maintenance Membership is a voluntary, fee-based primary care program focused on routine healthcare, prevention, medication management, health education, and ongoing communication with your provider.

When you establish care with Handsome Health, your healthcare is centered around an ongoing provider-patient relationship.
Your care may include:
Preventive Health Planning
We can work together to identify appropriate preventive screenings, laboratory testing, health goals, and other recommendations based on your individual health history and needs.
Medication Review
Medication management may include reviewing current medications, identifying potential concerns, monitoring therapy when appropriate, and determining whether prescription changes or refills are medically appropriate.
Stable Chronic Condition Management
Patients with stable medical conditions may receive ongoing monitoring and medication management when appropriate.
If your condition becomes more complex or requires a level of care outside the membership’s scope, you may be referred for additional evaluation or specialty care.
Laboratory Review
Laboratory testing may be recommended for preventive care, medication monitoring, or evaluation of a medical concern.
When results are available, your provider can review them with you and explain what they may mean in the context of your overall health.
Routine Follow-Up
Healthcare doesn’t end when you leave the appointment.
Depending on your needs, follow-up may involve an office visit, telehealth visit, telephone communication, or secure messaging.

Telehealth When Appropriate

Depending on the situation, telehealth may be used for services such as:

- Routine healthcare questions
- Treatment-plan follow-up
Telehealth cannot replace every in-person examination.
If your provider determines that an in-person examination, testing, imaging, or another level of evaluation is necessary, you may be asked to schedule an office visit or seek care elsewhere.
Telehealth services are provided within the provider’s authorized scope of practice and consistent with applicable Illinois requirements and standards of care. Illinois law permits healthcare professionals to provide telehealth within their authorized scope of practice and consistent with standards applicable to in-person care.
Important: For telehealth services through Handsome Health, patients should be physically located in Illinois unless otherwise permitted by applicable law and determined appropriate by the Practice.
Your Membership Is Not Health Insurance

The Handsome Health Health Maintenance Membership is a fee-based healthcare program, not health insurance.
Membership does not replace comprehensive health insurance coverage.
Membership fees generally do not cover expenses such as:
- Prescription medications
- Pharmacy charges
- Specialist visits
- Hospital care
- Emergency care
- Imaging
- Outside laboratory charges unless specifically included
- Procedures
- Services outside the membership
- Other healthcare services not specifically identified as included
Your specific benefits, fees, exclusions, cancellation terms, and other financial terms are governed by the Health Maintenance Membership Agreement.

Membership-based primary care is not a substitute for emergency medical care.
Secure messaging, telephone communication, and routine appointments are not continuously monitored for emergencies.
Do not wait for a response from Handsome Health if you are experiencing a medical emergency.
Call 911 or seek emergency medical care immediately for symptoms such as:
- Chest pain
- Severe difficulty breathing
- Stroke symptoms
- Loss of consciousness
- Severe allergic reaction
- Severe bleeding
- A rapidly worsening or potentially life-threatening condition
